How to Register as a Patient

Complete a Patient Registration Form

The registration process will take around 15 minutes to complete. You will need to complete the process in one session.

To enable registration you will be need to provide the following information:

  • A Registration Form completed with your NHS Number (we are unable to register you without your NHS Number, you can get this from your previous GP Surgery or from your repeat prescription).

When the form is completed bring it into reception to complete the registration process.

Temporary Residents

We are happy to see patients who are temporarily resident in the practice area should medical attention be required. In this instance persons wishing to be seen should contact the surgery by telephone and state that they are a temporary resident.

Completion of a Temporary Resident Registration form will be available from reception.

Named Accountable GP

All surgeries are required by the Government under the terms of the latest GP contract (2015 to 2016) to allocate all patients a named accountable GP.

Castlefields Surgery operates on a personal list basis therefore patients will be familiar with having a named accountable GP.

For convenience if you have a medical card your named accountable GP will be the named doctor you are registered with.  Patients registering over the past couple of years may not have received a medical card as these are no longer routinely sent out when registering with a new GP.  

The surgery keeps a record of your registered accountable GP and you will have been informed who that is when registering with the surgery.  There is no need to contact the surgery for this information.

If you need input from a variety of health professions, your registered accountable GP will be responsible for ensuring the oversight of your care.
